Program
Person Center Services believe that every person regardless of disability, deserves a safe and nurturing home. Our Community-based services which comprises of apartment homes, In-Home Support and Respite Services offer men and women with developmentally and physically disable the individualized services and supports needed in order to live independently within the community.
Supported Living Services (Community Living Services)
Person Center Services recognize persons, regardless of disability, desire to have meaningful choice and control of their daily lives. As a provider of Supportive Living services, Person Center Services team works with persons to encourage independence and community inclusion in an effort to reach their long-term personal goals while living in a supervised apartment. Support services may include assistance with activities of daily living, behavioral programs, integration into community life, and personal financial matters.
In-Home Support
In-home supports services are essential to ensuring that persons enrolled in the ID/DD Waiver continue to receive services and supports in the comfort of their own homes or family homes.
Companion
Companion services provide non-medical assistance and supervision to support a person’s goals, desires, and needs as identified in the person’s Individual Support Plan (ISP), and reflected in his or her Person-Centered Thinking and Discovery tools. Goals may be related to the person’s safety, promotion of independence, community integration, and/or retirement.
Respite Services
Respite care provides relief to the family or primary caregiver to meet planned or emergency situations. Respite care gives the caregiver a period of relief for scheduled time away from the individual, including vacations. It may also be used in case of emergencies. Respite is only provided to those individuals who live in their own home, or their family home.
